School Registration 2025-2026

For Registration questions, contact our Registration Hotline 847-327-7200 Monday through Thursday 7:30 a.m. - 4 p.m. Please make sure to complete ALL steps of registration.

Your registration is finished if you have:

Completed your student's online registration
Created a Parent PowerSchool Account (if you didn't already have an existing account)
Paid your registration fees via our web store
Provided Proof of Residency if required

Incoming freshmen and new students, don't forget to turn in your health, dental forms and order your PE Uniforms if you haven't already done so!

Step 1 - Online Registration

Current Students: Registration is open from Monday, June 2 through Thursday, June 26.

Students Transferring to LHS: Please call 847-327-7015 for more information before beginning the registration process.

Navigate to our Registration Gateway System to begin

Students entering Grades 9-12 your login information will be mailed to your home the first week of June.

Once you log in, click Edit to begin.
Scroll through each page and make sure that all information that we have on file is still current and update any information that is needed. Some information such as medical is required to be updated each year.
Make sure ALL parents are listed as contacts, even if they do not live in the same household. It is extremely important we have all contact information on file.

Step 2 - PowerSchool Account

New to D128?

Incoming freshman families should have done this in January/February.

Set up a D128 PowerSchool Parent Portal

A parent/guardian new to District 128 needs to create their own Power School account (A PowerSchool account from another school cannot be used )
Navigate to the D128 Parent PowerSchool Portal to create your account
Create a parent/guardian account by entering the information below

Your First and Last Name
Your email address

Choose a username and password (please note that usernames and passwords are case sensitive)

Already have a current D128 student?

Add students to your existing District 128 parent account if you currently have a student enrolled in D128. Skip this part if you have just created an account or already linked your student.

Log into your Parent Powerschool Account, click on the Account Preferences button located at the left hand menu
On the top left side, click on the Students tab
Click the Add button

Link Your Student to Your PowerSchool Parent Account

You need the Parent Access ID and Access Password for each student you want to link to your Single Sign-on. Please contact the registration hotline at 847-327-7200 if you do not have this information.

Step 3 - Pay Registration Fees / PE Uniforms

Log into your parent PowerSchool account and choose Balance from the left hand menu. Once your student's fees have been displayed, you may click on Make a Payment in the upper right of your screen and you will be redirected to the webstore to pay your fees.

You will need to set up a web store account if you are a new parent to LHS or if you have not previously used the web store.

- pre-orders thru August 30th with Registration. Order after this date can be made through Jostens.

All freshman and new students need to purchase two PE Shirts at this webpage.

Fee Waivers are available for families who qualify.

To apply for Financial Assistance, please complete the application.

Payment Plans are also available. To request a payment plan, please complete the application.

Step 4 - Proof of Residency

We digitally verify residency for D128 students.

Only the following situations need to provide additional documentation to verify proof of residency:

Families who we were not able to digitally verify residency. These families will be notified in the registration letter.
Current Parents/guardians who have moved/changed their address must re-verify residency.

All proof of residency forms need to be complete by June 28.

To verify your proof of residency, please fill out this form and upload 3 documents to the link provided in the form.

One document must be a mortgage statement, lease, or property tax bill
The other two documents should be bills (credit card, cell phone, water, cable etc)

Feel free to mark out any dollar amounts and account numbers, all that is necessary to see is the name of the company the bill is from, your name and address. All bills submitted must be within 90 days

Step 5 - Medical Forms Submission

All incoming 9th grade students must have a complete ISBE health physical, dental exam & immunization record on file prior to the first day of school. In addition, any student wishing to play a sport, needs a sports physical on file. Medical forms including physicals, asthma action plans, administration of medication etc can be submitted the following ways:

What should I do if I might qualify for a Basic Fee Waiver?

Parents who would like to apply for a Basic Fee Waiver need to complete the Fee Waiver Application located on the documents page and return it to the main office. If approved, the Basic Student Fee and Driver Education class fee (if applicable) will be waived.

All registration fees and outstanding fees must be paid or have made payment arrangements by Thursday, June 28.
