Registration Health Requirements
9th Graders
Physical Requirements:
9th graders:
- All students who will be 9th graders in 2025-2026 are required to submit a complete Certificate of Child Health Examination Form, dated on or after August 11, 2024, with complete immunization records. - A dental exam record must also be submitted, as mandated by the state of Illinois. The exam must have been completed between November 15, 2024 and May 15, 2026.
Any 9th grade student not in compliance with the physical and immunization requirements will be excluded from school beginning October 15th until the correct information is submitted to the nurse’s office. Dental exam forms are due by May 15th, 2026.
